Well, in Splashfighters, if you team kill, you get a RapidDemise/Foul Play prize, does that count?

 

Before they updated Runescape's bounty hunter gave you a rouge penalty if you didn't kill your target.

 

In Cabal,when you PvP a player too many times,you will not be able to play for certain number of days.(my friend got banned for 2 months)

 

In splashfighters, if you play sports mode and kill too many players, you get the Unfair Play Prize, which gives you negative points.

 

In 2moons/Dekaron, you can be sent to jail for killing too many players I think... or something like that anyway.

You get an 'Outlaw' Status which makes you have a higher chance of dropping items when you die, you also can't teleport or purchase from NPCs.

 

In dragon oath you can be sent to jail for a while, get a mark that makes you an easy target for people to kill and get items from you if they want, and sometimes you get stuck in jail for hours if you're bad enough

i dont know if this is correct but i used to play in Byond a game called Dragonball z dark apocalypse that allow ur character to make ur own title

 

Would Runescape count? Not only usernames but they had like a switch your name/title kind of thing so you could fit in your clan, or a group.

Also Combat Arms you get ranks for leveling up.

and so on. They also grant bonuses for you.

 

You get special titles for your character in GodsWar Online
